Investigation Summary
At approximately 12:15 p.m. on November 12, 2014, an employee had been controlling nearby traffic. The contractor was installing permanent electrical power distribution poles with a boom-type excavator. The contractor was lifting a pole out of a hole with a boom-type excavator. As the pole was lifted out of ground at approximately 45 degrees, it began to tip and fall. The employee had moved into the path of the falling pole for unknown reasons. The employee was struck by a 22-foot temporary wood power pole, weighing approximately five (500) pounds and was killed.
