Employee killed in 18 ft fall from roof

Employee #1 was attempting to climb approximately 18 ft 6 in. down from the roof of a building. The 15 ft portable aluminum ladder that he had used to gain access to the roof was positioned about 3 ft 6 in. below the roof landing. As the employee attempted to get over the parapet wall onto the unsecured ladder, both he and the ladder fell. Employee #1 sustained multiple fractures and was killed.
