EMPLOYEE #1, A 53-YEAR OLD LABORER, RESIDED AT 149 RUNNING BEAR MABANK, TEXAS, 75147, PHONE #(214) 451,3662. HE AND THE CRANE'S FLAGMAN WERE WORKING ON THE ROOF OF A THREE LEVEL PARKING GARAGE LOADING 2 BY 10 FOOT SCAFFOLD PLANKS. THE ROOF'S EDGE WAS NOT GUARDED, NOR WAS EMPLOYEE #1 EQUIPPED WITH ANY KIND OF PPE (SAFETY BELT, LANYARD, ETC.). EMPLOYEE #1 TRIPPED OR STEPPED OFF THE EDGE OF THE ROOF AND FELL 10 FEET, 6 INCHES TO A CONCRETE SURFACE. EMPLOYEE #1 DIED OF HEAD INJURIES.
