Investigation Summary
At approximately 11:00 a.m. on November 3, 2014, an employee was standing on a small scaffold that was at the edge of a second story balcony while installing tongue and groove ceiling planks in the balcony ceiling. The employee fell 21.5 feet from scaffold and balcony. The employee sustained head and spinal cord injuries. He was killed from his injuries.
