Investigation Summary
At approximately 7:30 p.m. on January 7, 1989, Employee #1 was riding in the back of a pole truck. He was standing behind the driver and holding onto the headache rack. The truck's hook was not snubbed to the rear of the truck and the winch was engaged without anyone realizing it. The winch pulled the hook up to the top pulley and then pulled the A-frame poles over the top. The poles struck Employee #1, killing him.
