Employee killed when trapped by falling piece of casing

Employee #1 was working and observing in an area in which a coworker was operating a Texoma hole digger. When the auger caught a piece of concrete it moved the concrete into a 25 ft piece of hole casing that was leaning against an excavation wall. When the 25 ft casing moved up, a 12 ft piece of casing lying on top of it fell over onto Employee #1. That casing section trapped him between it and a small 3 ft piece of casing that he was standing over. Employee #1 was killed instantly.
