Investigation Summary
A patient in a Neurology ward became combative and attempted to leave the ward. Employees #1, #2, and #3 attempted to retrain the patient. The patient pushed all three employees through a lowered ventilation panel. The employees and the patient fell 30 ft through the ventilation shaft to a concrete floor. Employees #1 and #2 were killed. The patient and Employee #3 were seriously injured. (The original report lists 4 employees as being killed or injured, however, it is unlikely that the injured patient was an employee.)
