EMPLOYEE IS CRUSHED BY FORKLIFT WHILE UNLOADING PIPE
EMPLOYEE #1 WAS WORKING ABOARD A SHIP ON A FORKLIFT UNLOADING STEEL PIPE. WHEN THE PIPE SUDDENLY SHIFTED, EMPLOYEE #1 JUMPED FROM THE LIFT TRUCK AND RAN IN FRONT OF IT. THE PIPE FORCED THE TRUCK FORWARD AND PINNED EMPLOYEE #1 TO THE WALL, CRUSHING HIM TO DEATH.
