Employee killed when head is pinned by forklift forks

Employee #1 was helping the HEB International foreman to clean up the company yard. At approximately 12:10 p.m., Employee #1 began using a 25 ton forklift to move 7 5/8 in. by 38 ft pipes. At approximately 12:20 p.m., the foreman went to wash his truck. Upon returning at approximately 12:45 p.m., he found Employee #1 lying on a rack of pipes with the left side of his head pinned under the right fork of the running forklift. The foreman immediately lifted the fork loaded with pipes, turned off the forklift, and checked Employee #1 for a pulse. Employee #1 was dead.
