Investigation Summary
Employee #1 was attempting to speak to another employee who was working at the grit blasting station. This station consists of a skid in which a 5,000 lb, 33 in. diameter steel pipe is grit blasted in preparation for painting and coating. The pipe is moved by the skid and rolls into the grit blaster by the control station, which is run by an operator. Employee #1 attempted to cross through the machine while it was running and was hit in the chest, knocked down, and hit in the head. The machine was a Wheelabrator two upblast pipe cleaning machine, serial #A125743. The pipe that struck the employee was a 31 3/8 inch, 45 foot length steel pipe.
