EMPLOYEE #1 WAS IN A TRENCH, WORKING CLOSE BEHIND A DITCH DIGGER (DITCH WITCH). HE PLACED HIS FOOT TOO CLOSE TO THE TEETH ON THE DIGGER AND WAS CAUGHT AND PULLED INTO THE ROTATING DIGGER TEETH. HE WAS KILLED.
