Employee suffers a heart attack and dies
Employee #1 and two coworkers were manually lifting a 250 lb I-beam that was to be installed as part of an overhead rail system. While Employee #1 was holding one end, he experienced chest pains and thought he had strained a chest muscle. He went to the clinic, where he suffered a massive heart attack and died.
