CONSTRUCTION EMPLOYEES KILLED/INJURED BY FALLING EQUIPMENT

EMPLOYEES #1 AND #2 WERE WORKING NEAR A SIFTER AGITATOR WHICH HAD RECENTLY BEEN MOVED USING A MANUAL LIFT TRUCK. THE UNIT HAD BEEN LEFT IN AN UNSTABLE POSITION. A HEAVY ROTOR SECTION OF THE AGITATOR BEGAN TO ROTATE AS THEY ATTEMPTED TO STABILIZE IT (IN PREPARATION OF ANOTHER MOVE), CAUSING IT TO TIP OVER. EMPLOYEE #1 WAS CRUSHED TO DEATH. EMPLOYEE #2 WAS HOSPITALIZED WITH INJURIES.
