EMPLOYEE HIT BY FALLING HOIST BLOCK, FELL AND FRACTURED HEAD

EMPLOYEE #1, A CRANE OPERATOR, WAS LIFTING A LADLE OF MOLTEN METAL FROM A FURNACE PIT USING THE MAIN HOIST OF A REMOTE CONTROLLED, 75 TON, P 1/2 H OVERHEAD CRANE. A SECOND RADIO FREQUENCY UNIT IS USED TO MOVE HEAT CURTAINS AROUND A FURNACE ADJACENT TO THE ONE SERVICED BY THIS CRANE. EMPLOYEE #1 WAS STANDING AT THE EDGE OF THE PIT IN FRONT OF THE LADLE, WHERE HE COULD SEE BOTH LIFTING LUGS OF THE LADLE. ANOTHER EMPLOYEE WAS STANDING TO HIS LEFT WAITING TO HOOK UP THE DOUBLE LEG SLING ON THE AUXILIARY HOIST TO THE BOTTOM OF THE LADLE. EMPLOYEE #1 WAS LOWERING THE MAIN HOIST LOAD BLOCK, WITH SPANNER BAR AND HOOKS, TOWARDS THE LIFTING LUGS OF THE LADLE WHEN THE AUXILIARY HOIST, DIRECTLY ABOVE HIM, WAS ACTIVATED. THE LOWER BLOCK OF THE AUXILIARY HOIST WENT THROUGH THE UPPER LIMIT SWITCH AND PARTED THE HOIST CABLE. THE BLOCK AND CHAIN SLING FELL TO THE FLOOR. EMPLOYEE #1 WAS HIT ON THE LEGS BY THE BLOCK AND SLING. HE FELL BACKWARDS, FRACTURING HIS SKULL. THERE WERE PREVIOUS INSTANCES WHEN THE UPPER LIMIT SWITCH HAD NOT FUNCTIONED WHEN THE RADIO FREQUENCY EQUIPMENT HAD BEEN ACTIVATED FOR NO APPARENT REASON.
