Employee killed when head struck by beam

Employees placed two hydraulic jacks on either side of a 22 ft long wooden, glue-laminated beam to take the weight off the vertical supports and to move the beam laterally 1 inch. The employees moved the beam into place and were ready to lower the jacks when the beam moved off the temporary vertical support and struck Employee #1 in the head, killing him.
