EMPLOYEE KILLED WHEN FORKLIFT FELL ON HIM

AT APPROXIMATLEY 10:40 AM ON FEBRUARY 24, 1988, EMPLOYEE #1 WAS OPERATING A STAND-UP TYPE LIFT TRUCK WHICH IS ENTERED BY THE OPERATOR THROUGH THE OPEN REAR AREA. HE HAD POSITIONED HIS TRUCK ON A RAISED DOCK LEVELER WITH THE REAR OF THE LIFT TRUCK FACING AN EMPTIED TRAILER. THE LEVELER LIP WAS SUPPORTED AT ITS OUTER EDGE BY THE REAR OF THE TRAILER. PREPARATIONS WERE UNDERWAY FOR THE DEPARTURE OF THE TRAILER. SUDDENLY AND WITHOUT ANY WARNING THE TRACTOR TRAILER PULLED AWAY FROM THE LOADING DOCK, LEAVING THE OUTER LEVELER LIP UNSUPPORTED. THE DOCK LEVELER, WITH THE LIFT TRUCK ON IT, DROPPED TO ITS LOWEST POSITION (BELOW DECK LEVEL). THE LIFT TRUCK, WITH EMPLOYEE #1 STANDING AT ITS REAR, FELL TO THE DRIVEWAY. THE OPERATOR (EMPLOYEE #1) WAS CRUSHED TO DEATH WHEN THE LIFT TRUCK FELL ON HIM.
