Employee killed when struck by machine parts under pressure

Employee #1 was inflating a tire on the air-feed shaft of a Cincinnati buffing machine, model 50A, serial #100488, when the threaded portion of the shaft failed. The bolt sleeve and retaining plate were propelled with great force due to the sudden pressure release. Some or all of the pieces struck Employee #1 in the head, causing a serious concussion. He was killed.
