EMPLOYEE #1 WAS STANDING ATOP A TEN FOOT MOBILE LADDER STAND AFTER DOING SOME WORK ON A MACHNE. HE FELL DOWN THE STEPS, SUFFERING A FATAL HEAD INJURY. THE CAUSE OF THE FALL COULD NOT BE DETERMINED.
