Employee killed in fall with ladder

At 7:15 a.m. on June 10, 1986, Employee #1 met with coworkers in the tool room on the third floor. At 7:30 a.m., Employee #1 and one of the carpenters that he was assisting went down the stairs to the second floor to install a wall. The carpenter told Employee #1 that they needed an extension cord. Employee #1 went back to the tool room, got an extension cord, plugged it in, and dropped it down to the carpenter. The carpenter acknowledged the cord and turn to talk to a coworker. A secured wooden ladder from the third to second floor was within 20 ft of the tool room. An unsecured aluminum ladder was directly in front of the tool room. Between 7:35 and 7:40 a.m., Employee #1 attempted to descend the aluminum ladder but both he and the ladder fell from third floor to the second floor. Employee #1 regained consciousness and was rushed to the hospital, where he died on June 20, 1986.
