EMPLOYEE #1 WAS GREASING GEARS WHILE THE GEARS WERE IN OPERATION. THE EMPLOYEE'S HAND OR ARM BECAME CAUGHT IN AN INGOING NIP POINT. HIS ENTIRE BODY WAS PULLED INTO THE GEARS AND HE WAS KILLED.
