FELL FROM, AND STRUCK BY, POWERED INDUSTRIAL TRUCK

EMPLOYEE #1 AND A FORKLIFT OPERATOR LOADED SOME TIRES ONTO A FORKLIFT TRUCK AND HAULED THEM TO THE NORTH END OF THE SALVAGE YARD. ALTHOUGH THE FORKLIFT OPERATOR CLAIMED THAT EMPLOYEE #1 WAS SITTING ON TOP OF FORKLIFT'S COUNTERBALANCE WEIGHT, HIS EXACT POSITION ON THE FORKLIFT IS NOT CLEAR. HE APPARENTLY FELL OFF THE FORKLIFT AND WAS RUN OVER BY IT. HE WAS NOTICED LYING FACE-DOWN ON THE DIRT ROAD, BEHIND AND TO THE RIGHT OF THE FORKLIFT. HE COMPLAINED OF BREATHING PROBLEMS AND DIED ON THE WAY TO THE HOSPITAL. THE MEDICAL EXAMINER CLAIMED THAT THERE WERE NO OBVIOUS SIGNS ON EMPLOYEE #1'S BODY OF HAVING BEEN RUN OVER, BUT HE DIED FROM A MASSIVELY LACERATED LIVER, A BRUISED URINARY BLADDER AND A BRUISED RIGHT LUNG.
