EMPLOYEE KILLED IN FALL FROM SCAFFOLD

AT APPROXIMATELY 8 AM ON SEPTEMBER 29, 1986, EMPLOYEE #1, A LABORER, WAS APPARENTLY WALKING ALONG A SCAFFOLD, 23 FEET ABOVE THE GROUND. (THERE WERE NO WITNESSES TO THIS ACCIDENT.) HE WALKED INTO THE AREA WHERE 2 PLANKS BRIDGED THE 33 INCH GAP BETWEEN THE SCAFFOLD AND THE FIRST FLOOR OPEN-SIDED FLOOR EDGE (NEAR WALL LINE K). HE APPARENTLY EITHER SLIPPED OR TRIPPED AND FELL 3 1/2 FEET TO THE BRICKLAYER'S FOOT PLANKS BELOW THE GAP. HE THEN APPARENTLY ROLLED THROUGH THE 16 TO 17 INCH UNPROTECTED WALL OPENING BETWEEN THE BOTTOM OF THE UPPER WALL OPENING AND THE FOOT PLANKS (ON THE SCAFFOLD SIDE ARM BRACKET). HE THEN FELL 6 FEET, 10 INCHES TO THE LOBBY FLOOR CONCRETE EDGE, OUTSIDE THE PERIMETER CABLES. HE APPARENTLY BOUNCED OFF THAT FLOOR EDGE (THERE WAS BLOOD IN THE AREA) AND FELL THE REMAINING 12 FEET TO THE GROUND. HE LANDED ON HIS BACK BETWEEN THE SCAFFOLD AND THE FOUNDATION WALL.
