EMPLOYEE CRUSHED BETWEEN STEEL COIL AND A GENERATOR HOUSING

THE ACCIDENT OCCURRED AT THE L.T.V. STEEL COMPANY, INC.'S COLD ROLLING MILL, LOCATED APPROXIMATELY 1 MILE EAST OF HENNEPIN, ILLINOIS, AND EAST OF STATE ROUTE 71. EMPLOYEE #1 WAS STANDING 8 FEET TO THE EAST OF THE COIL INFEED CONVEYOR UPENDER, NEAR THE 500 KW GENERATOR AT THE SOUTHEAST CORNER OF THE 83 INCH COLD TEMPERING MILL. A 58,000 POUND, 83 INCH DIAMETER, 34 INCH WIDE STEEL COIL FELL OFF THE UPENDER AND ROLLED THROUGH 2 PARTLY SECURED HAND RAILS, OVER AN ELECTRIC CONTROL BOX ON 2 STEEL POSTS AND CRUSHED EMPLOYEE #1 AGAINST THE SOUTH EAST EDGE OF THE GENERATOR HOUSING.
