ON JANUARY 27, 1987 AT APPROXIMATELY 9:15 AM, EMPLOYEE #1 WAS CLEANING THE INSIDE OF A TANK TRUCK WHILE STANDING ON TOP OF THE TANK. HE SLIPPED AND FELL 15 FEET, STRIKING HIS HEAD ON A STEEL BEAM. HE DIED AS A RESULT OF THE INJURY.
