EMPLOYEE CRUSHED BETWEEN TAIL GATE AND TRUCK BODY

AT APPROXIMATELY 2 PM ON DECEMBER 8, 1986, EMPLOYEE #1 WAS ATTEMPTING TO ACTIVATE A "STUCK" ELECTRIC/HYDRAULIC TAIL GATE LIFT, LOCATED AT THE REAR OF AN ENCLOSED STAKE BODY TRUCK. THE EMPLOYEE WAS FOUND WITH HIS HEAD CRUSHED BETWEEN THE ASCENDING TAILGATE AND THE TRUCK BODY. THERE WERE NO EYEWITNESSES.
