EMPLOYEE #1 WAS CLEANING INSULATORS IN AN ELECTRICAL VAULT WHILE STANDING ON A FIBER GLASS LADDER, 15 FEET ABOVE THE GROUND. HE JUST FELL BACKWARDS FROM THE LADDER, STRIKING A PIPE ON THE WAY DOWN BEFORE HITTING THE GROUND.
