Investigation Summary
AT APPROXIMATELY 11:45 AM ON JANUARY 17, 1987, EMPLOYEE #1 WAS USING A TRIPOD MOUNTED SURVEYOR'S LEVEL TO SHOOT ELEVATIONS FOR STRUCTURAL STEEL COLUMNS. HE HAD POSITIONED THE LEVEL ON A BARE "I" BEAM. HE FELL APPROXIMATELY 14 FEET, LANDING ACROSS A LOWER LEVEL "I" BEAM. HE SUSTAINED HEAD, CHEST AND ABDOMINAL INJURIES FOR WHICH HE WAS HOSPITALIZED. HE LATER DIED IN THE HOSPITAL. FALL PROTECTION DEVICES WERE NOT USED. A SAFETY BELT, LANYARD AND LIFE LINE WERE NOT USED, NOR WERE GUARD RAILINGS IN PLACE.
