APPARENT HEART ATTACK WHILE OPERATING FORK LIFT

AFTER EMPLOYEE #1 HAND LOADED SOME SALT BAGS ONTO A TRUCK, HE BOARDED HIS FORK LIFT, TOYOTA-TYPE G, S/N-40FG18106-97, AND DROVE BACK INTO THE WAREHOUSE TO RETURN THE PALLET TO ITS STACK. HE RAISED THE PALLET TO MAKE A RIGHT TURN TO RETURN THE PALLET. THERE WAS A LOADED PALLET ON THE FLOOR BEYOND THE POINT WHERE EMPLOYEE #1 WOULD BE TURNING NORTH TO RETURN THE PALLET TO ITS STACK. INSTEAD OF TURNING NORTH, THE FORK TRUCK CONTINUED WEST, STRIKING THE PALLET ON THE FLOOR. THE FORK TRUCK CAME TO REST WITHIN 2 FEET OF ANOTHER FORK TRUCK, TYPE YALE S/N M222201. APPARENTLY EMPLOYEE #1 BEGAN TO EXPERIENCE HEART ATTACK SYMPTOMS JUST BEFORE HE WOULD HAVE TURNED NORTH. AFTER HE HE STRUCK THE PALLET ON THE FLOOR, HE FELL FROM HIS FORK LIFT AND STRUCK HIS HEAD ON THE PARKED FORK TRUCK. HE CAME TO REST ON THE FLOOR WITH HIS LOWER BODY ENTANGLED IN HIS FORK LIFT. CPR WAS ADMINISTERED BUT TO NO AVAIL.
