Investigation Summary
Employee #1 was unloading the sixth of seven new Pontiacs from an auto transport carrier at a dealership. He was using a different carrier from the one he had used for at least four years, which had the foot walks removed. Employee #1 fell approximately 9 ft to the asphalt pavement, fracturing his head and right leg, and was found dead about 3 minutes after he was last seen. The car door was open. Hydraulic fluid, generated by a nearby hose fitting, created a slipping hazard on the fixed ladder the employee was using. The anti-slip grating on top of the carrier was damaged, creating a tripping hazard. The convertibles on the carrier had been driven on front-first, not backed on to prevent the back windows from blowing out. This positioned the cars so that the doors, when opened, inhibited access to the top of the carrier, creating a barrier hazard. Ninety-nine percent of the time the cars are backed onto the carrier. Employee #1 had not been provided follow-up training since he was hired 29 years ago.
