Investigation Summary
At the end of the workday, a flagging crew was knocking over the signs that had warned traffic of the distance to a highway paving project. After the first set of signs had been knocked down, Employee #1 got back on the tail gate of the pickup truck to ride to the next set. He fell from the tail gate of the moving pickup truck and sustained fatal head injuries. There was no safety belt, lanyard, or other restraint system in place at the time of the accident. It is possible that Employee #1 was dizzy as a result of constant exposure to the paver's fumes, but the cause of his fall is unknown.
