Abstract: At approximately 8 a.m. on March 16, 2012, Employee #1, a 46-year-old male, mechanic with Shadow Holdings Llc Dba Bocchi Laboratories was performing the duties of a mechanic, including cleaning a pneumatic valve. Employee #1 had started to clean a valve manufactured by Sudmo, for not more than one minute before the incident occurred. Employee #1 asked a coworker, for his assistance to clean the valve. Employee #1 connected the valve to the Kaps-All "Filler" machine's (Serial Number: 6733) air signal in line-four where it could be actuated. Employee #1 gave the order to the coworker to actuate the valve at the control switch and commented to him that it opened. The coworker then turned the control switch to close the valve. Employee #1's fingertip was amputated as he stuck his fingers in the valve to begin cleaning it. The supervisor was not in the immediate area. Employee #1 suffered an amputation of his left ring fingertip when it was pinched by a pneumatic valve. Employee #1 was transported to Henry Mayo Hospital for treatment. The facility involved in the incident is a manufacturer of personal care products including shampoos, creams, and body lotions, and employs approximately 340 persons.
