Abstract: At about 6:00 p.m. on, or about, August 27, 2013, Employee #1, an electrician with Grimmway Enterprises, Inc., and a coworker (a maintenance mechanic) were working on a roof-mounted swamp cooler. Employee #1 was an active party in the repair of the cooler. The employees had replaced the bolts on the electric motor so that they could place tension on the drive belt that they had replaced. Then, Employee #1 began conducting an amp test on the electric motor. The lockable fuse disconnect door was open, in order to conduct the amp test. The cooler manufacture was Champion Cooler (Date of Manufacture: February 2003; Serial Number: LC65658; Model Number: 6500DD). The causal factor of the accident was that there was a bare energized 480 volt, three-phase wire that Employee #1 came into contact with. Employee #1 died at the scene.
