Abstract: At approximately 3:30 a.m. on April 9, 1999, an employee received serious injuries while working in the cereal division of a food manufacturer. The employee was working on a Kelly Duplex Horizontal Mixer, C-49. He had finished a dry-fruit cereal batch and was preparing to clean the mixer. The employee turned the power off on the control panel, but did not lock the power switch in the off position. After cleaning the mixer at the elevated work platform, he went downstairs to remove the left-over material in the outlet area of the mixer. He removed the chute from the outlet of the mixer. As he put his hand into the outlet of the mixer, he heard "a click" and the machine started running. The employee's right hand became caught in the moving parts of the mixer resulting in severe lacerations and multiple fractures. The employee stated that he did not know who turned the machine on. Other witnesses stated that the machine was in the running mode when the victim was removing the material from the mixer.
