Abstract: At 9:00 p.m. on April 16, 2000, Employee #1 was attempting some unidentified activity near the tail spool of an incline conveyor that was used to move onions into a process for the sorting out of debris, such as rocks. While doing this activity, Employee #1's hand was drawn into the point where the in-running part of the conveyor belt meets the tail spool, pulling Employee #1 face first into the tail spool where he was pinned. Employee #1 suffered an amputation and died of his injuries.
