Abstract: At 12:05 p.m. on July 5, 2006, Employee #1 was working as a temporary or secondary employee for So Cal Bakeries, which was dba Gold Coast Baking Co. He had been sent there by his primary employer, L&T Staffing, Inc., dba Staffing Solutions. Employee #1 was working as a machine operator at the employer's facility, a bakery. He was operating the intermediate proof box, with serial number L4-INT- 01, on the "K" Line. It made bread dough balls. He started his shift at 10:30 a.m. and continued working on the machine until 11:30 or 11:45 a.m., when he took his break. He said that when he left, the guard on the chains and sprockets was in place. When Employee #1 came back from his break at 12:00 noon, he resumed working on the machine. As he leaned to his left, his left hand was caught in one of the three chain and sprocket drives, while the chains were in motion. The index finger of his left hand was amputated, and the third, fourth, and fifth fingers (middle, ring, and small fingers) of that hand were lacerated. He was hospitalized. There were no witnesses to the accident.
