Abstract: On May 5, 2002, Employee #1 was setting up equipment for a production run on Line #3, near the oven. There were two conveyor belts located in the same location, one was a metal conveyor and the other a belt conveyor. Sometime during the set-up, the metal conveyor reversed in the opposite direction, creating a pinch point between the belts. Employee #1 got his right hand caught between the belts and used his left hand to pull out his right hand. Both hands were caught, with the right hand receiving the most serious injuries. Employee #1 was working alone at the time of the accident. When other employees attempted to stop the machine, the emergency stop located on the opposite side of where Employee #1 was located did not function. So, another employee pulled a yellow rope successfully stopping the belt conveyor. However, the metal conveyor was still operating and pulling in Employee #1's hand. Employee #1 managed to break his left hand free and reached the controller box to turn off the metal conveyor. He sustained serious injuries to his right hand. He had a deep second to third degree burn to his proximal right palm and a superficial friction burn to his distal right palm. There were also minor abrasions on his left palm. Employee #1 was transported to Fountain Valley Hospital and transferred and treated at Western Medical Center in Santa Ana, California. He was released that same day.
