Abstract: At approximately 3:50 a.m. on June 8, 2000, Employee #1 was troubleshooting flight chains that were hanging up a donut line conveyor belt. Employee #1 and the gravey, who was a foreman, were looking at the box former while it was running to see where the problem was. Employee #1 crouched down in a catcher's position, lost his balance, and began to fall forward. He put his hands in front of him to catch himself and both his hands got caught in the moving chains. Employee #1 suffered partial amputation of the index and middle fingers of both hands.
