Abstract: On July 12, 2011, Employee #1, a sanitation worker of Marquez Brothers International, Incorporated, was cleaning a CRYOVAC Rotary Vacuum Chamber machine located in CP-1 at the Hanford, CA facility. The sanitation process required the machine to be in operation. Employee #1 was authorized to access and clean the enclosure housing and the belt and pulley drive/s for the feed conveyor/s. For some unknown reason, Employee #1's left hand came into contact with the belt and pulled though the belt and pulley drive. Employee #1 was transported to the hospital and treated for the amputation of his pointer and middle fingers. Employee #1 was experienced working in CP-2, but was reassigned to CP-1, thirty days prior to this incident. It was found that the equipment and machines in CP-2 were equipped with direct drives motors, while the CRYOVAC CP-1 was equipped with belt and pulley drives and was the only machine identified in the facility required to be in operation during the sanitation process. Upon reassignment, Employee #1 was not retrained, nor was documentation provided for hazards associated with belt and pulley drive systems. The employer did not establish specific procedures and training in accordance with 8CCR Section 3314. The belt and pulley drive was determined not be effectively guarded as required by 8CCR 4070(a) based on the enclosure's door not being secured as required by 8CCR Section 3942.
