Abstract: At 6:26 a.m. on June 4, 2002, a serious injury occurred at the Blue Banner Company on their wash line. The employer is engaged in the packing of citrus fruits. The employer had 135 employees and was non-unionized. At the time of the accident, Employee #1 was pushing grapefruits in a soak tank that was 28-feet long, 6-feet wide and 27-inches deep, by using a paddle. While pushing the grapefruits, her right arm shirt sleeve got caught in a rotating shaft, approximately 5-feet long, 1.2-inches in diameter, located 32-inches above the floor, and turning at 102 rpm. Employee #1's right arm got entangled with the shaft, which resulted in multiple fractures of her right arm and she was hospitalized for treatment.
