Abstract: Employee #1 was a member of the night cleanup crew at Agri Empire's potato processing plant, assigned to the processing area in shed #1. At approximately 10:40 p.m. on March 19, 1996, he was cleaning weeds out from under the conveyor belt at the end of conveyor #7 when his right hand became entangled in the belt and conveyor system and was crushed as it was drawn through the roller and sprocket at the end of the conveyor. Employee #1 sustained severe crushing injuries to his right hand, multiple lacerations over the dorsum of his hand, and a fracture of the ring finger metacarpal. At the time of the accident, conveyor #7 was energized and running, and the belt and sprocket at the end were unguarded. If the company's lockout/blockout procedures had been applied during the cleaning operations, the moving belt would not have drawn in Employee #1's hand. If the belt and sprocket at the end of conveyor #7 had been properly guarded, his hand could not have entered the zone of danger.
