Abstract: On February 24, 1996, Employee #1 was working on a Kenco crimping press, model 18-209, serial #68-007, that was equipped with a two-hand trip device and an anti-repeat feature. Due to a machine malfunction, the slide came down unexpectedly while Employee #1 was positioning a part. Her right hand became caught in the die at the point of operation, amputating part of her little finger. Her ring finger also sustained deforming injuries. Employee #1 was taken to the University of California at Irvine Medical Center for treatment.
