Abstract: On April 23, 1999, Employee #1 was operating a Rockwell RT-40 Table Saw located in the Box Shop of Building 106. He was in the process of cutting 6-in. strips from various sized pieces of plywood. According to Employee #1, the blade was set at a height of approximately 3.25 in. above the table. The guard was in place and he was wearing gloves. Before the accident occurred, he had cut about 40 pieces and indicated that during the times when he was cutting 1-in. strips, the pieces were falling into the "gap" on the left side of the blade insert plate. He stated that he used the "push stick" to remove the pieces out of the opening in the plate insert. According to Employee #1, when the accident occurred he was in the process of using the push stick to remove a 1-in. strip of wood out of the gap on the left side of the blade in the insert plate when the guard flipped up and his right gloved hand made contact with the running saw blade. Employee #1 suffered deep lacerations to his index finger and the amputation of the first joints on his right middle and ring fingers. Employee #1 was taken to the hospital.
