Abstract: At approximately 11:00 a.m. on January 14, 1998, Employee #1 worked the early morning shift as cleanup laborer in the slice and dice department of a fruit processing plant (large apple peeling machines). Employee #1 had been in that department for approximately five years. The apple peeling machines would sometimes jam. Employee #1 had observed the mechanic un-jam them by turning off power and pulling on the belt. The mechanic was not in the area, so Employee #1 had another employee turn off the machine, and then he pulled the belt in an attempt to un-jam it. Employee #1 had the other employee turn the machine back on. Employee #1's left little finger got caught in the belt and pulley drive, amputating it past the knuckle. Employee #1 was trained in lockout/block out; however his job duties did not include maintenance.
