Abstract: At approximately 4:45 p.m. on January 4, 2012, Employee #1, with Nemo's Bakery, Inc., was at a commercial bakery cleaning an icing pump (Waukesha, Model Number 030, Serial Number 235011 R199) that was used to deposit icing onto cinnamon rolls. With the pump plugged into an electrical source, he flushed the pump of residual icing with water from a hose. Employee #1 then unscrewed the pump body port and stuck his left index finger inside the pump throat to determine if any icing was lodged inside. His finger contacted the rotor blade and was partially amputated. An attempt was made to reattach the amputated portion of the finger, but the effort was not successful.
