Abstract: At approximately 12:00 p.m. on August 24, 2010, Employee #1 was working for a plumbing company at a residential home construction project. He was standing on a 6 ft tall A-frame ladder drilling a hole in a wooden top plate between the first story ceiling and the second story floor of the building. The top plate where the employee was drilling was approximately 9 ft, 8 in. above the concrete, ground level floor. Coworkers in a nearby room heard a thudding noise and found the employee lying unconscious on the concrete ground near the corner of the room, where he had been drilling. The employee had fallen from the ladder. Employee #1 was taken to a local hospital, where he was hospitalized and treated for severe brain injury and subdural hematoma. The reason for the fall was unknown, as the worker was alone in the room at the time of the accident, there were no eyewitnesses, and the employee had no recollection of the incident.
