Abstract: At approximately 4:30 p.m. on August 23, 2006, Employee #1 and a coworker were painting a kitchen of a single family home. Before work began, the supervisor brought two five gallon buckets of white wood undercoat and one five gallon bucket of lacquer thinner to the jobsite. The supervisor then instructed Employee #1 and the coworker to use for painting with a spray assembly. The kitchen of the residence was masked off from the other portions of the house creating a tent-like atmosphere. As Employee #1 was spraying underneath a wall mounted oven, an explosion occurred. A fire ball formed and struck the back kitchen wall, igniting the plastic sheeting used for masking off the area. Emergency services were contacted, and Employee #1 was taken to UCSD Burn Center, where he was hospitalized and treated for serious burn injuries. The accident investigation revealed that the explosion occurred due to an oven pilot light that was the source of ignition. The flammable vapor of the paint allowed the atmosphere to exceed the allowable Lower Explosive Limit (LEL), causing the explosion when introduced to the ignition source. The atmosphere was not tested prior to the explosion.
