Abstract: At approximately 10:50 a.m. on April 23, 1999, Employee #1, a bakery worker, was working in the Proof Box Area. Employee #1 was loading proofing racks 28.5 in. wide by 77 in. tall with trays of bread dough and placing those racks into the proof box. Employee #1 opened the door to the proof box to roll one rack inside. As Employee #1 was rolling the rack to the door, the door began to close and caught his left hand between the door and the side of the proofing rack. The first distal portion of Employee #1's second finger was crushed and later amputated.
