Abstract: On December 4, 2004, Employee #1 was working at a facility that manufactured cookies and crackers of a wide range and variety. It was a newer plant with modern factory layout and excellent housekeeping. Employee #1 was working at conveyor line Number 3. It was a 4-ft-wide and approximately 200-ft-long wire-mesh conveyor that included an oven for cookie drying. At the discharge end of the oven, under the conveyor, there was a motor-driven sprocket drive. The sprocket was guarded by a sheet metal casing from the outside, as seen while looking from the adjoining walkway. On the opposite side of the casing, under the conveyor, the sprocket was exposed. The casing dimensions were 14.25 ft by 5.5 ft. Employees were performing the weekly sanitizing operation of the line. Employee #1, who was a sanitizing mechanic, was wrapping the electric motor, next to the sprocket drive, with a plastic sheet so that water would not get inside. Employee # 1 put his finger inside the casing. The index finger and third finger on his left hand were amputated. Employee #1 was hospitalized.
