Abstract: At approximately 11:25 p.m. on June 18, 1997, Employees #1 and #2, challenge course facilitators, connected their safety harnesses to the zip wire rope to descend after sending students through the double zip wire activity. They connected to the zip wire pulley improperly and when the two employees started down, the zip pulley failed and they fell approximately 25 ft to the ground. Employee #1 died at approximately 8:30 a.m. on June 22, 1997 from injuries sustained in the accident. Employee #2 was hospitalized until June 30, 1997. The employees violated company policy and training by double zipping and by not attaching the sling lines to ensure a proper connection to the zip wire.
