Abstract: At approximately 11:48 a.m. on March 7, 2001, Employee #1 was putting down a hot tar roof and was stapling down a base material. The roof had several openings covered by wooden pallets. The wheels of the staple gun ran into one of the pallets and Employee #1 moved the pallet partially back. As he rolled the staple gun pass the opening, he fell 22 to 26 ft to a concrete floor. He was hospitalized for multiple broken bones and a head trauma.
